Coaches Kit Order Form

Each coach will receive a kit package of the following:

• 1x Polo Shirt
• 1x Zip Top
• 1x Tracksuit Bottoms
• 1x Rain Jacket

This is subject to renewal every two years.

ALL ITEMS ARE SUBJECT TO STOCK LEVELS AT NIKE AND COULD BE SUBSTITUTED IF UNAVAILABLE
